Tendonitis and swelling of the knee

Trauma to the knee

After a knee trauma, the knee will swell very quickly, often due to bleeding in the joint (hemarthrosis). The injured structure inside the knee (meniscus, ligament-in particular, the anterior cruciate-or bone in the case of a fracture) will bleed and cause an increase in intra-articular pressure.
In the case of trauma that has damaged the surrounding muscles, the swelling will be more lateral, as the blood will accumulate around the muscles.
Generally speaking, any trauma affecting the knee can lead to swelling. It is also true for surgical interventions such as installing a prosthesis. However, in this case, the swelling is “normal” and expected (it will disappear several months after the operation).
 

Osteoarthritis and arthritis of the knee

Osteoarthritis of the knee can also be responsible for swelling. It is progressive, not very marked, or of variable appearance (it appears and disappears at intervals). Also, the intensity of the swelling may vary from one day to another, depending on the activities.
 
The knee will also swell in cases of arthritis (the knee is the joint most affected by septic arthritis). As in the case of hemarthrosis, the knee will be swollen, red, hot, and very sensitive:
 
In the case of septic arthritis, the bacteria that may be involved are staphylococcus aureus (mainly), streptococcus, and even pneumococcus.
In the case of aseptic arthritis, the pain will be less intense.

Infections that cause swelling

Many infections can also cause swelling of the knee. It is, for example, the case with tuberculosis, syphilis, etc. Similarly, Lyme disease can be involved in swelling of the joints, particularly the knee.
 

Swelling of the knee in pregnant women

Swelling of the knees can also be observed in pregnant women. Indeed, by increasing the uterine pressure, the baby compresses the vena cava responsible for bringing blood from the lower limbs to the heart. It results in water retention, which can sometimes be observed in the knee.
 
 

Symptoms of a swollen knee

Swelling of the knee is a symptom in itself, and it is vital to look closely at it to determine its origin. Usually, this swelling is accompanied by pain, especially in the case of rapid swelling following a trauma. Hemarthrosis of arthritis is characterized by a red, warm knee (due to inflammation) and sometimes bruising.
 
If it is aseptic arthritis, the pain will be less severe than in septic arthritis. They will start gradually, increasing, and will be particularly marked in the night’s second half. It should be noted that popliteal cysts do not cause any pain.
 
A synovial effusion is responsible if the knee tends to swell at the end of the day or very gradually. This increase in synovial fluid can also occur in the hours or days following trauma. The swelling is then less significant than in the case of hemarthrosis. It is not this effusion that causes the pain. On the other hand, the swelling shows that the joint is suffering.
